Members pay a reasonable annual fee, low $12 appointment fees, and get a 25% discount on diagnostic, restorative, preventative, and some cosmetic services.

Dental services included in the plan include:
  • Two periodic exams
  • Two bitewing x-rays per member during each enrollment
    period
  • Two $35 cleanings
